To receive a merchandise certificate, all of the following must be sent to the address shown postmarked no later than May 31, 2012: this completed request form; proof-of-purchase from the charger; and the original, dated cash register receipt or invoice from your purchase of either the ElectriFly TritonEQ or Triton2 EQ charger included in this offer between February 1 and April 30, 2012. This is a manufacturer offer and cannot be redeemed by retailers or combined with any other offer. Requests from retailers, groups or organizations will not be honored. Duplicate submissions will be discarded. Limit one merchandise certificate per purchase.
